Reliable transportation for SNF residents

From dialysis runs to specialty consults to hospital transfers, DachiPlus is the steady transportation operation your social work and nursing teams have been looking for.

DachiPlus provides transportation contracts for skilled nursing facilities across Nassau and Suffolk Counties: discharge coordination (same day when a vehicle is available), recurring dialysis runs, specialty consults, and hospital transfers. Driver HIPAA privacy training is in progress, vehicles carry commercial livery insurance as New York State requires, and DachiPlus is MAS-credentialed for resident Medicaid billing. Call (516) 754-7777 to discuss a facility contract.

Trip types

What we transport for SNFs

  • Recurring dialysis (3x weekly standing orders)
  • Oncology and infusion therapy
  • Specialist consultations
  • Diagnostic imaging (MRI, CT, X-ray)
  • Hospital transfers (inbound and outbound)
  • Wheelchair, stretcher (sub-contracted), bariatric

Discharge coordination: how social workers use us

When a resident is cleared for discharge, the social worker or case manager calls DachiPlus dispatch: a direct facility contact, not a general booking line. We confirm vehicle type, pickup time, destination address, and any specific resident needs (mobility devices, oxygen, companion). Typical dispatch time from the discharge call is under 30 minutes for scheduled discharges, and under two hours for same-day requests when a vehicle is available. Call as soon as the discharge time is known. We tell you on the call whether a same-day ride is available.

At the destination (home, another SNF, or an outpatient rehab facility), the driver assists with the handoff and notifies facility staff of arrival. For transfers to another care facility, we coordinate directly with the receiving facility's admission team if needed.

Recurring routes for SNF residents

Many SNF residents have recurring transportation needs: dialysis three times a week, weekly oncology appointments, regular specialist visits. For Medicaid residents, the New York State Department of Health's Standing Order Policy (effective October 1, 2020) allows only the resident's medical provider or a designee, such as an RN, LPN, or scheduler, to request a standing order, by entering it on the MAS provider portal. Transportation companies cannot enter, change, or pause standing orders, so DachiPlus does not set them up. When your team enters one, list DachiPlus in the preferred transportation provider field. Each standing order lasts up to six months and does not renew on its own, so plan renewals before it ends. For private-pay residents, DachiPlus manages recurring schedules directly with the facility. Either way, once the trips are assigned to us, DachiPlus handles every trip without a new call for each one.

Driver continuity is built into recurring routes wherever possible. When a resident knows their driver by name, it reduces boarding anxiety and makes the trip smoother for everyone.

What SNF compliance requirements DachiPlus meets

DachiPlus vehicles carry commercial livery insurance, as New York State requires. For facilities that require vendor documentation as part of their compliance process, DachiPlus provides:

  • Driver background check documentation
  • HIPAA privacy training program documentation
  • NY State DMV registration documentation
  • MAS credentialing confirmation for Medicaid billing

How to set up a facility transport contract

Call (516) 754-7777 and ask for facility partnerships. We'll review your census, typical trip types, and billing pathways: Medicaid, private-pay, Medicare Advantage, and long-term care insurance. Most SNF accounts are active within 48 hours of initial contact. There's no minimum trip commitment to get started.
