## How do Medicare Advantage transportation benefits work?

Medicare Advantage plans must cover everything Original Medicare covers, but they can add extras. Transportation is one of the most common extras, especially in plans designed for chronically ill members or low-income beneficiaries (D-SNPs and C-SNPs).

## What do Medicare Advantage plans typically cover?

- **Trip count:** A set number of one-way trips per year — commonly 24, 36, 48, or 60.

## The allowance is counted in one-way trips, not journeys

This is the detail that most often causes a Medicare Advantage transportation benefit to run out earlier than the member expected. Plans that include transportation typically grant a fixed number of one-way trips per plan year — a figure in the twenties or thirties is common, though it varies widely — and a round trip to an appointment consumes two of them. A member reading "24 trips" reasonably hears two dozen appointments; in practice it is twelve, and fewer still if any of them involve a stop on the way.

Two related rules are worth checking at the same time. The allowance almost always resets at the plan year rather than rolling over, so unused trips are lost rather than banked. And coverage is usually limited to plan-approved destinations, which in practice means in-network medical facilities — a trip to a specialist outside the network may be a perfectly good medical decision and still not be a covered ride.

## Booking through the wrong channel is the expensive mistake

Most Medicare Advantage plans administer this benefit through a designated transportation vendor and require the trip to be arranged through that channel, usually with a day or more of notice. Telephoning a transport company directly and hoping to claim afterwards generally does not work: if the trip was not authorised through the plan's process, it does not count against the allowance and it is not reimbursed, which leaves the member holding a private-pay charge they did not expect. Book through the plan first, every time, even when a direct call would be quicker.
